All information you receive is based on evidence based practice and although we endevour to be able to help and provide improvements to all our patients we make no gaurantee’s we will be able to fully resolve your childs concern.
The General Data Protection Regulation (GDPR) is a law regarding how personal data is processed in the UK. It applies to anyone who processes data as part of their business, and requires transparency about how your information is processed.
This policy is effective as of 25th July 2019.
Why we collect your personal data?
“Lucy Jackman RD” will only collect necessary data for patient care. We collect personal data with consent from you, to be able to provide a dietetic service to you.
What data do we collect?
Personal data related to new patient registration form
Information regarding your child's health, medical history and nutritional / feeding history as provided to us during consultations
Any further clinical information that is provided to us from any other health care professional
This is for use of Lucy Jackman, registered dietitian only for patient identification purposes. Lucy Jackman, registered dietitian is responsible for ensuring all personal data is handled and stored in accordance with GDPR data protections laws.
What counts as information?
Personal information includes handwritten and electronic notes, completed patient registration forms, questionnaires and pre-consultation documents given to us. Please be aware this also includes letters, emails, text messages and whatsapp messages.

Professional clinical information keeping for client information
Sharing with other named health care professionals involved in your child’s care. Please note any information will only be shared with your consent unless we deem the child or yourself to be at risk from a safeguarding perspective and it is in the best interests if the patient.
Safeguarding your personal information:
“Lucy Jackman RD” is committed to ensuring that your personal information is secure. In order to prevent unauthorised access to information or disclosure we have ensured that all information is stored in an encrypted electronic system.
Your personal information and clinical assessment notes are stored on password protected file on Microsoft Onedrive Cloud, only accessed from a password protected computer. Any handwritten clinical notes taken during your consultation, have all identifiable information removed and are kept in a securely until transferred onto the electronic file. All handwritten notes will be shredded.
Any emails, text messages or whatapp messages containing personal, identifiable information will will be protected using the security password. All passwords will only be known to Lucy Jackman and will not be shared at any point.
Sharing your personal information:
We are committed to ensuring your information and privacy is always protected, therefore communication is always private and confidential or encrypted (where possible). If this is not possible, your full name will not be used as a means of identification.
With your consent information maybe shared about you by the following methods:
Over the telephone, only in a private location
Depending on your preference, we may also communicate by text message or whatsapp.
Your information will only be shared with appropriate parties on a need to know basis; this is in keeping with all aspects of GDPR and the British Dietetic Association code of ethics. With your consent and if required, your information may be shared with the following parties:

Social and welfare organisations
Family, friends and other represatitives during the consultation
We will not distribute, sell or lease your personal information to third parties unless we have your explicit permission or are required by law to do so.
You may request details of personal information, which we hold about you under the General Data Protection Regulation (GDPR). If you would like a copy of the information held on you please contact Lucy Jackman. If you believe any information we hold for you is inaccurate or incorrect, please contact Lucy Jackman and we will promptly correct this information.
Under GDPR you have the right for your information to be deleted at any point on request. However, please be aware of the Department of Health’s legal and statutory requirement that information is kept for a minimum of eight years; after that time frame your information will be deleted.
